Hi. My name is Dr. Tahir, and I am an orthodontist in the Oak Lawn and Berwyn area in Illinois. One of the main reasons I wrote this report is because I commonly get the question
When should braces or Invisalign start for my child?
There are so many different theories out there, and it can get so confusing - but it doesn’t have to be that way.
Here’s a shocking fact:
It’s not just you and the general public that’s confused, but often your general dentist and your orthodontist don’t know the best time for starting braces or Invisalign treatment, either!
How do I know this? Because I was confused myself as an orthodontist, until I figured out that starting braces or Invisalign too soon can cause more problems than it solves.
Now I want to help you and your child to understand how braces or Invisalign work best - and how NOT to get trapped into what I call the “ten year braces or Invisalign plan.”
What I’m about to tell you is going to clear up a lot of confusion about so-called ‘early braces treatment, Phase 1 treatment or interceptive treatment’.
I know my methods differ from a lot of the other orthodontists out there - and I''m OK with that. Instead of recommending to my younger patients, "Let''s get started!", I often say, "Let''s wait a bit."
Instead of convincing parents to start ''early intervention'' treatment that often gives no (to very little) added benefit for most children, I warn them about the problems with starting braces too early for their child.
Or using devices to stretch the jaws so that every crowded tooth can be fitted in. Or to start taking out teeth just because a child is a certain age and they don’t have all their adult teeth in. Or just because a child has an overbite
I don''t believe in starting treatment before your child is truly ready, and I stand by my methods - in my almost 2 decades of being in the orthodontic field, the principle of NEVER starting braces too early has stood the test of time.
You see, during my university specialty training at University of Illiniois at Chicago, I was fortunate enough to observe both methods of starting braces, since we had both a group of teachers who believed in early treatment and a group who did not.
Because of their differing philosphies, one group would begin braces as early as possible, while the second group would wait, starting almost all of their cases AFTER all or most of the adult teeth were in - regardless of the age of the patient.
After I graduated from orthodontic school, it took me a few years to realize that nearly every case of early braces treatment for children is a waste of time, money and energy. There is simply no real advantage to starting braces for most children while their jaw and teeth are rapidly changing, growing and forming! It''s great CONCEPT but REALITY is different!
I have seen patients who were promised that if they did early treatment, full braces might not be needed later. Their parents loved to hear this because they saw savings of money and time and the possibility of no braces for their children in the future.
Unfortunately, that’s usually not how things worked out.
You see, I even said the same things in my early years during my own patient consultations, thinking I was doing the right thing.
Eventually, however, I realized that I ended up putting braces on almost all of my patients anyway (after the so called Phase I treatment) because all of their teeth did not come in straight.
That kind of took the wind out of my sails. Isn’t the point of treatment with braces is to have even, straight teeth? I realized what common sense should have told me all along that starting treatment on a mixture of baby and adult teeth was almost always going to end in some correction but not FULL correction and the need for full braces will be there for nearly all children- simply because there was no guarantee the rest of the adult teeth would come in straight!
How could I possibly promise my younger patients that early treatment would keep them from needing braces later?
I started losing sleep over the fact that what I was telling my patients and what I was actually SEEING didn’t match up. I simply couldn’t keep my promises that early braces treatment would solve their problems.
I changed my approach many years ago, and became one of the few orthodontists I know who encourages patients to WAIT (till the right time).
Now I want to save you and your child the headache of being in braces or Invisalign for too long and spending a lot of extra money to get your treatment done – when everything can be done in ONE PHASE by just waiting for the RIGHT TIME.
I know there’s a huge temptation to ’get it over with’.
Especially in America, where looks are so important, there is enormous pressure to have early treatment started in hopes that a child can avoid the possibility of having to wear braces in high school. The problem is, most kids have major growth and body changes during high school, meaning after all that work and worry they end up in braces anyway.
There’s so much confusion and misinformation about early orthodontic treatment out there that you can talk to three different orthodontists and get three different answers!
For example, suppose you have a nine year old child with poorly spaced teeth and an overbite.
One orthodontist tells you,
“Now is the perfect time to correct the overbite and close up those spaces, because the body is developing rapidly. This means any changes we make now will be better and more permanent - and maybe no braces will be needed later.”
Unfortunately, this probably isn’t true. In 98% of cases, your child will STILL need braces later.
The second orthodontist tells you,
“It’s best to use braces and headgear now so that the overbite gets smaller and spaces close up. You need to understand, however, that more work will be necessary later when all her adult teeth come in.”
This is true - but do you hear what’s really being said?
You are being told “Pay for early braces treatment now - and be prepared to pay for more treatment later.” Why would you want to do that?
The third orthodontist (ME) tells you,
“Let’s hold off a bit and let her adult teeth come in first, so we know what we are really working with. A lot can change in a few short years, and by waiting we can correct everything at one time.”
All of these different opinions can be really confusing to someone trying make the best decision for their child.
You need to know the details about WHY it’s best to wait to start braces.
