This happened to my brother. He has mental challanges. But was sent every six months for exams. For one I found out too late that he was a night grinder of his teeth. He was having more and more tooth pain. Finally Dr Schupanitz said there was nothing else he could do for him. So I got involved and made an appoimnet in Iowa City> Once there we found he had not only (which we could see, and Dr would put fillings in them) ground down his front teeth. But his whole back area had an actual tunnel ground in his teeth with raw nerve showing. I personal feel he should have been truned in. Thank God he's no long praticing. I have tried to understand this whole mess. But this is pure neglance!
