I am a European living in Tucson. My dental insurance is United Concordia, and my current 12-month benefit period started on 02/01/09. During my previous benefit period (02/01/08 to 01/31/09) I underwent extensive dental work at Dr. Leong's office. My insurance payments reached the $1200 max. towards the end of 2008. At that point, I explicitly agreed with him and his secretary to take a break and to complete the remaining dental work "next year, at the beginning of my next benefit period". Then I told his secretary that I was not sure about the exact start date of my next benefit period, and asked her whether she could possibly look it up for me, and to schedule my next appointment after the that date. She answered "Certainly!" and scheduled me for a crown on 01/15/09. However, unbeknownst to me, that date was still within my PREVIOUS, already maxed-out benefit period! Trusting her, I showed up at the appointment and paid my projected co-pay (i.e., 50% of the full charge). Now, however, I have received a letter from my insurance company stating that they will not pay for that crown because it was placed during my previous period. If she had paid attention and scheduled my appointment just two weeks later, I would have saved $460.
