With my chart on the computer screen right in front of him, Pawluk was about to prescribe a medication to which I am allergic. There were also 2 medications listed as ones I am allergic to and I am not. I pointed this out but it was never corrected. Promised follow-up from the office staff required 3 phone calls. Important information was omitted in my care instructions. I would not return to this practice under any circumstances.
