I have been going to see Dr. Doty since I was 3 years old. He does wonderful work, and is very conscious of the patient's needs. There is never any nagging about the patient's hygiene, just gentle reminders to do better. (They don't scold you like a little kid.)
It's a relaxed atmosphere, and even though they are out of my network, I still am willing to pay the extra for the great care I receive there.
