Scaling and root planing is performed on patients with periodontal attachment loss, inflammation, subgingival calculus deposits, and diseased root cementum. It is considered active therapy, as opposed to being preventive in nature.

A procedure known as "subeptithelial connective tissue graft," which is billed on a per tooth basis. Connective tissue is typically harvested from the palate of the donor, by making two closely spaced incisions and removing the tissue between them. That tissue is typically used to cover exposed tooth roots, preventing sensitivity and caries; and increasing the zone of keratinized tissue to resist food abrasion and toothbrushing forces over time.
